Dear __________:

It has been three weeks since we last met. You have missed two scheduled appointments and have not responded to the telephone messages I have left for you. I am hoping you will contact me in the near future so we can make arrangements for your ongoing psychotherapy. As we have discussed during the course of your psychotherapy, regular attendance at treatment sessions and active follow-through with treatment recommendations are of great importance. While I know you have made progress in our work together I strongly believe that abruptly discontinuing your psychotherapy is not in your best interest.

I am hoping you will contact me so that treatment may continue. If for some reason you have decided not to continue your treatment under my care I still request that you contact me to discuss this decision and so we may make the appropriate arrangements for your ongoing treatment elsewhere. If you decide to discontinue all treatment despite my recommendations I am hoping you will at least call me so we can discuss your plans. My clinical experience has demonstrated that meeting for at least one final session can help to make for a better outcome.

If I do not hear from you within three weeks of the date of this letter I will consider your treatment under my care to be terminated. Please remember to only use medications under the close supervision of your prescribing physician. Also, if you experience any difficulties and are not receiving your needed psychotherapy you may go to your local hospital's emergency room or you may call the County Hotline at (111)-111-1111.

I hope to hear from you soon, but if I don't, I wish you success in your ongoing endeavors.
