This is the Alzheimer's section of the National Institute on Aging of the NIH. The 9 Fact Sheets (under Publications) are solid information booklets and a good starting place.

Practice Guidelines for the Treatment of Patients with Alzheimer's Disease and Other Dementias of Late Life

This is the Practice Guideline from the American Psychiatric Association. It is a mini-textbook and the sections on features and natural history, treatments, and treatment planning may be useful.
